Oligozoospermia is a clinical condition characterized by a subnormal concentration of spermatozoa in the ejaculate. According to the World Health Organization (WHO) reference limits, a normal semen sample must contain at least 15 million spermatozoa per milliliter (or a minimum total output of 39 million sperm per ejaculate).

Sperm Concentration Classification:

Etiology and Root Causes:

1. Pre-Testicular Factors: Endocrine dysregulation within the HPG axis, including secondary hypogonadism, hyperprolactinemia, and exogenous anabolic steroid exposure.

2. Testicular Factors: Intrinsic testicular pathologies like varicocele (dilated veins causing hyperthermia and oxidative stress), cryptorchidism, and toxic exposures.

3. Post-Testicular Factors: Partial ejaculatory duct obstruction (EDO) or partial seminal tract blockages.

Diagnostic Follow-Up Logic:

  • Rule of Biological Variation: A single subnormal test does not establish a permanent diagnosis. Repeat the semen analysis after a 2- to 4-week interval following 2–7 days of abstinence.
  • Endocrine Panel: Check serum Total Testosterone, Free Testosterone, FSH, LH, and Prolactin. An elevated FSH suggests testicular failure; low FSH/LH suggests central hypogonadotropic hypogonadism.
  • Scrotal Doppler Ultrasound: Assesses testicular volume, microcalcifications, and confirms venous reflux (varicocele).
  • Genetic Screening: Mandatory when sperm concentration is <5 million/mL (Karyotyping and Y-chromosome microdeletions).
