Antisperm Antibodies (ASA) mediate immunological infertility when immunoglobulins (IgA, IgG, IgM) target surface antigens on spermatozoa. A test is considered clinically positive when ≥ 50% of motile spermatozoa are bound to antibody particles in standard Mixed Antiglobulin Reaction (MAR) or Immunobead tests, creating agglutination and preventing oocyte penetration.

Diagnostic Workup Logic

  2. Direct MAR Testing: Mixing fresh, motile semen with antibody-coated latex particles and observing for particle binding.
  3. Serum Indirect ASA Testing: Testing the female partner's serum or male seminal plasma when direct testing is inconclusive or when unexplained agglutination is present.

Etiological Stratification & Blood-Testis Barrier

Naturally, the blood-testis barrier (BTB) sequesters developing sperm cells from the body's immune surveillance.
