Sperm agglutination describes motile spermatozoa sticking to one another (head-to-head, tail-to-tail, or mixed). It must be distinguished from sperm aggregation, which refers to immotile sperm adhering non-specifically to cellular debris, leukocytes, or mucus strands, which has lower clinical significance.

How to Read Your Semen Analysis: Agglutination Grades

Agglutination Grade Microscopic Description Clinical Relevance & Fertility Impact
Grade 1 (Isolated) Less than 10 sperm per clump; many free-swimming sperm seen. Often clinically insignificant; natural conception remains highly possible.
Grade 2 (Moderate) 10 to 50 sperm per clump; free-swimming sperm are still present. Reduces progressive motility; sperm may struggle to reach the egg.
Grade 3 (Severe) Greater than 50 sperm per clump; very few free sperm observed. Significantly impairs spontaneous conception; requires immediate diagnostic workup.
Grade 4 (Complete) All motile sperm are glued in massive tangles. Prevents progressive movement into the cervical mucus; causes functional subfertility.

Etiological Stratification

  • Antisperm Antibodies (ASA): Immunoglobulins binding motile spermatozoa via specific surface receptors, gluing them together.
  • Bacterial Surface Adherence: Microorganisms (such as E. coli) adhering to sperm membranes, causing flagellar cross-linking and clumping.
  • Altered Seminal pH & Glandular Secretions: Dysfunctional accessory gland secretions (prostate or seminal vesicles) altering local electrical charges on sperm membranes.
