A varicocele is an abnormal dilatation of the pampiniform venous plexus draining the testicle. Present in approximately 15% of the general male population, it is found in 35% to 44% of men with primary infertility and up to 45% to 81% of men with secondary infertility, representing the most common correctable cause of male factor subfertility.

- Scrotal Hyperthermia: Venous blood pooling elevates scrotal temperatures by 1.5–2.5°C, destroying temperature-sensitive enzymes required for normal spermatogenesis.
- Adrenal/Renal Metabolite Reflux: Left internal spermatic vein enters the left renal vein at a 90-degree angle, causing hydrostatic backpressure. This leads to left-sided varicoceles in over 85–90% of cases and reflux of toxic metabolites.
- Radical Assault (ROS): Severe hypoxia in testicular tissue triggers high reactive oxygen species, leading to lipid peroxidation and DNA breaks.
Guidelines for Surgical Varicocele Repair (AUA / ASRM / EAU)
Varicocele repair is indicated when: 1) the varicocele is palpable on physical exam; 2) the couple has documented infertility; 3) the female partner has normal or treatable fertility; and 4) the male partner has abnormal semen parameters or elevated sperm DNA fragmentation (SDF). Treatment is not indicated for subclinical varicoceles detected only by ultrasound without a palpable abnormality.
